Catholic Education of W.A. Form 11

When you arrive at a new Catholic Primary School for the first time in 2024, you must present the following required CEWA Form 11.  Complete your relevant section and take it with you to the new school.

We recommend scanning the Form after you've completed your sections, keep a copy on file and ready to take to any new Catholic School that you attend.
